Very disappointed how this office is working now. I have been a patient there for over 10 years and was very good. Sadly I can't say the same now I was coming for my regular anual check up and my appointment was cancel due to the hurricane Irma understandable. However I rescheduled then I was called same day before my appointment that the midwife had a labor ok then they gave me another appointment for next day I showed up there and the other midwife had a surgery and was going to take over an hour to come over. Seriously I spent time from work coming over for nothing.

- You turned away a patient that booked a month in advanced that showed up 20 minutes early for her appointment because of this issue, putting the blame back on her for not filling out the thing you never sent.

A good friend recommended Dr. Johnson to me after he delivered her two children. He is without a doubt the best OBGYN in Ocala. His staff is friendly and professional and I never feel like an inconvenience or as if I'm just a number like I've experienced at other OBGYN's. Dr. Johnson genuinely seems to care about his patients. I wish he was a primary care physician!!!

I have been seeing Dr. Johnson since early 90's for my Gym care and OB care when he delivered my son in 2002. I have not seen another provider and do not plan on going anywhere. He is caring, professional and always willing to answer questions. I highly recommend Dr. Johnson.

I called them after finding this out to get clarification and they put the blame back on us saying that she should have received the letter and that they have electronic records so that questionnaire needs to be filled out ahead of time. I told them that they didn't give her that information at the time she booked her appointment and we didn't receive a letter and asked if they sent an email with the information, which they said they couldn't.

Dr. Lee delivered our beautiful baby boy today. I highly recommend her to any new parent. Her bedside manor is AMAZING, she is HUMOROUS, & most importantly very informative. We will be returning back to Dr J office for sure.

They said that she should have received a letter with a user name and password, which she never received, in order to fill out the questions. They tried to reschedule her, but at that point she would have to figure out when she could take the time off from work and start the whole process over again.

Needless to say we booked somewhere else.

- You have no means of your patients filling out this information at your office (ie tablets, computers) if this wasn't accomplished.

Wife booked an appointment here over a month in advance. She took a day off from work in order to have this appointment done. They told her when she booked her appointment that she would have to go online to fill out some forms in advanced. We went on their website the night before and downloaded and filled out the forms for her to bring to her appointment.

- You are a paperless only office that relies on your patients getting a PAPER letter in the mail to fill out your REQUIRED questionnaire.

I work in the medical field and if my office had policies like this, with this amount of disorganization, we would be out of business in a year.

Long story short, here are the things wrong with this story.

- Your website makes ZERO mention of this questionnaire on it whatsoever. There is a section for "download patient forms" with a bunch of paperwork to fill out in advanced. No mention of the questionnaire or even a heads up to call the office in advanced of you don't receive a letter, nothing.
